Otsuka Pharmaceutical has announced the publication of results from a randomized, double-blind, placebo-controlled clinical trial evaluating the efficacy of S-Equol, a naturally occurring soy-derived compound, in women with premenstrual syndrome (PMS) and premenstrual dysphoric disorder (PMDD).
Published in The Tohoku Journal of Experimental Medicine on July 30, 2026, the study suggests the potential usefulness of S-Equol-containing food in women with severe PMS or PMDD symptoms that do not require immediate medical treatment, contributing new scientific evidence to support women’s health.
PMS and PMDD Continue to Affect Women’s Quality of Life
PMS affects an estimated 80–90% of women of reproductive age to varying degrees and is characterized by recurring psychological and physical symptoms that develop several days before menstruation and improve after its onset. PMDD is the most severe form of PMS and is associated with marked emotional distress, impaired daily functioning, reduced quality of life (QOL), and decreased work productivity. While pharmacological therapies remain an important treatment option for moderate-to-severe disease, increasing interest in nutritional and self-care strategies has encouraged research into food-derived compounds that may help manage symptoms.
Randomized Trial Enrolled 119 Equol Non-Producers
Conducted in collaboration with Kindai University, the study enrolled 119 equol non-producing women aged 20–45 years who experienced PMS or PMDD symptoms that were not severe enough to require immediate medical treatment. Participants were randomly assigned to receive either an S-Equol-containing food providing 10 mg of S-Equol daily or a matching placebo for three months. Symptom severity was evaluated using the Daily Record of Severity of Problems (DRSP), an internationally recognized and validated tool widely used to assess PMS and PMDD symptoms through daily symptom recording.
Primary Endpoint Missed, but Subgroup Findings Suggest Potential Benefit
For the primary endpoint, change in total DRSP score from baseline, women receiving S-Equol experienced greater overall symptom improvement than those receiving placebo. However, the difference did not reach statistical significance. Subgroup analyses demonstrated that participants with more severe baseline symptoms showed findings suggesting the potential usefulness of S-Equol in women with severe PMS or PMDD. Although the primary endpoint was not statistically significant, these findings support further investigation of S-Equol as a nutritional approach for women experiencing severe premenstrual symptoms.
S-Equol May Offer a Nutritional Approach for Equol Non-Producers
S-Equol is produced when intestinal bacteria metabolize the soy isoflavone daidzein into a compound with estrogen-like biological activity. Previous collaborative research between Otsuka and Kindai University demonstrated that women who are unable to produce equol naturally have a higher risk of developing PMS and PMDD. The present trial is the first randomized, double-blind, placebo-controlled study to rigorously evaluate S-Equol supplementation alone in women with PMS and PMDD.
Study Supports Continued Research into S-Equol
Although the primary endpoint was not statistically significant, the findings provide important scientific evidence supporting continued investigation of S-Equol as a nutritional approach for women experiencing severe premenstrual symptoms.
