The surgical procedure of liver transplantation is very complex; it is often used to treat patients with last-stage liver disease. From the various types of liver transplants, living donor liver transplant (LDLT) is s special form that involves the partial removal of a healthy donor’s liver that is to be transplanted into the receiver. Though LDLT gives benefits, it also carries some generally manageable risks to both donor and recipient. The potential risks include some surgical complications, infections, organ dysfunction, and in very rare cases may lead to mortality. The recent case in Pune, India at Sahyadri Super Speciality Hospital which includes death of liver donor and recipient within few days of surgery represents very extraordinary and remarkable concerns in India’s medical history. The case underlines the inherent complexities and threats that can be involved in live donor liver transplantation (LDLT) and it also highlights the need of meticulous surgical protocols, extensive patient care and regulatory supervision to minimize the occurrence of adverse effects.
Case study
A 49-year-old male patient Bapu Komkar was diagnosed with end-stage liver cirrhosis. He was first presented to Sahyadri Hospital’s Deccan branch outpatient department on August 4, 2025. The medical team counselled him about the critical stage of his liver disease and suggested liver transplantation as the necessary treatment. His wife, Kamini Komkar, aged 42 ready to volunteer to be a living donor. She underwent thorough evaluation and clearance as per compliance with the statutory protocols mandated under the Human Organ Transplantation Act (HOTA). Both medical and ethical approvals for the transplant were got, with formal permission granted for transplantation on August 11, 2025.
The liver transplantation surgeries for both the donor and recipient were conducted sequentially on August 15, 2025, by multidisciplinary teams. First Kamini underwent a lobe hepatectomy (the precise lobe) during which all types of standard protocols including the preoperative, intraoperative and rigorous care during the transfer step were taken care of. Bapu received the liver graft with surgical, anaesthetic, and postoperative care that was conducted according to the institutional protocols.
Even though successful liver transplantation surgery has been carried out; the postoperative results were very tragic. Bapu’s condition weakened rapidly and he went in to cardiogenic shock and succumbed to death within couple of days. On the other hand, Kamini initially after the surgery showed good signs of recovery and was considered fit for discharge. But on August 21, she suddenly developed hypotensive shock followed by multi-organ dysfunction, which despite aggressive intensive care treatment, led to her death on the same day.
The hospital attributed Bapu’s death due to complications of his advanced liver condition and Kamini’s death as a sudden postoperative infection with resultant multi-organ failure. Autopsy and histopathological findings are pending to thoroughly confirm causality.
Hospital Response
Sahyadri Hospital acknowledged receiving notices from health authorities regarding the deaths of both the liver transplant recipient and the donor. The hospital stated that they are cooperating with the investigation carried out by the Maharashtra Health Department and Pune Municipal Corporation. Following the incidence, Sahyadri temporarily suspended its living donor liver transplant program but continued other surgical and medical services. The hospital denied any procedural failures or negligence and stressed on that all medical protocols and ethical standards were followed during preoperative counselling, surgery, and postoperative care. Hospital also highlighted the natural risks of liver transplantation, dragging attention towards recipient’s critical condition before surgery. Sahyadri has requested a fair and neutral review of the case before any disciplinary action is taken.
Government and Regulatory Perspective
The Maharashtra Health Department suspended Sahayadri’s live donor liver transplant license based on the Human Organ Transplantation Act (HOTA), mandating cessation of all live liver transplant surgeries pending. Also an eight-member expert committee was constituted under the chairmanship of Prof. Mohamed Rela, which is considered as a global authority in liver transplantation. This committee includes hepatologists, transplant surgeons, intensivists, infectious disease experts, and regulatory officials, tasked with investigating the clinical and procedural aspects within a four-week timeline.
The eight member expert committee will be evaluating all the medical records, treatment protocols, surgical video logs, ICU care notes, and autopsy/histopathology reports. The committee will examine if there were any type of violations of protocol, deficiencies in care, or negligence in any role happened. Pune Police have registered an accidental death case and are coordinating with Sassoon General Hospital, which will conduct an independent medical panel review as part of the investigation process. Family members have lodged formal complaints claiming medical negligence against hospital. The final call will be depends on the expert committee’s findings.
Expert Committee Members
The state government has formed an eight-member panel led by Prof. Mohamed Rela, an internationally renowned liver transplant surgeon. Other members include Dr. Ram Prabhu (liver transplantation, KEM Mumbai), Dr. Rahul Pandit (intensivist), Dr. Vasant Nagvekar (infectious diseases), Dr. Vijay Vohra (liver transplant anesthesia), Dr. Akash Shukla (hepatologist, KEM Mumbai), Dr. Padamsen Ranbagale (Sassoon General Hospital, Pune), and Dr. Bhagwan Pawar (Deputy Director of Health Services, Pune). This committee will investigate the medical, surgical, and procedural aspects of the case.
Going forward the suspension on live donor liver transplant remains in effect until an expert committee submits its detailed report. Committee, led by Dr. Nitin Ambadekar, Director of Health Services, is charged with conducting a thorough investigation within a four-week timeline. Their work includes formal hospital visits, detailed verification of medical and surgical records, staff interviews, and consultations with ancillary experts. Post-mortem toxicology and histopathology reports are awaited to clarify the precise causes of death for both patients.
In the long term, this case will likely trigger significant changes within Maharashtra and Indian Health System. New Guidelines will likely to be incorporate and safer methods of risk evaluation, pre-surgery counselling and mandatory clear documentation will be apply. Robust follow-up monitoring of patients after transplant to identify early issues can be implemented along with emergency response plans as standard practice. Initiatives toward greater public awareness of risks of living donations, and promoting organ donation by deceased donors may emerge as a way of easing the pressure on living donor programs and making patient protection and safety better.
