Eisai announced on October 15, 2026, that new oncology findings from the Phase 3 LITESPARK-011 and LITESPARK-012 studies will be presented at the European Society for Medical Oncology (ESMO) Congress 2026 in Madrid, Spain, from October 23 to 27. Both studies evaluate lenvatinib-based combination regimens in advanced renal cell carcinoma (RCC), but address different treatment settings.
The planned presentations include the final analysis of LITESPARK-011, an assessment of health-related quality of life (HRQoL) and treatment efficacy, and findings from LITESPARK-012. The results may add evidence on treatment outcomes after immunotherapy and investigational approaches for first-line therapy.
LITESPARK-011: Improving Disease Control After Immunotherapy
LITESPARK-011 (NCT04586231) is a Phase 3, randomised, open-label trial comparing WELIREG (belzutifan) plus LENVIMA (lenvatinib) with cabozantinib in patients with previously treated advanced clear-cell RCC (ccRCC) whose disease progressed following a PD-1 or PD-L1 inhibitor, with or without previous VEGF-targeted therapy.
The published second interim analysis demonstrated a statistically significant improvement in progression-free survival (PFS) with belzutifan plus lenvatinib. Median PFS was 14.8 months, compared with 10.7 months with cabozantinib, representing a 30% reduction in the risk of progression or death (hazard ratio [HR], 0.70; 95% confidence interval [CI], 0.59–0.84; one-sided p<0.0001).
However, the overall survival (OS) results were less conclusive. Median OS was 34.9 months with the combination and 27.6 months with cabozantinib. The difference did not reach statistical significance (HR, 0.85; 95% CI, 0.68–1.05; one-sided p=0.061).
These findings indicate improved disease control but do not establish a statistically significant OS benefit in the reported analysis.
FDA Approval and Safety Considerations
On September 24, 2026, the U.S. Food and Drug Administration (FDA) approved belzutifan plus lenvatinib for eligible adults with advanced RCC containing a clear-cell component following a PD-1 or PD-L1 inhibitor.
The FDA-reported prespecified interim analysis showed median PFS of 14.6 months with the combination versus 10.6 months with cabozantinib (HR, 0.74; 95% CI, 0.61–0.89; p=0.001). The objective response rate was also higher, at 53% versus 40%. However, the final OS analysis did not meet the threshold for statistical significance.
The PFS estimates differ slightly from those in the published second interim analysis because they reflect different analyses. They should be interpreted in their respective contexts rather than treated as interchangeable estimates.
Safety remains an important consideration. In the FDA-reported LITESPARK-011 data, serious adverse reactions occurred in 54% of patients receiving belzutifan plus lenvatinib. Reported serious reactions included hypoxia, pneumonia, hyponatraemia, acute kidney injury and cardiac dysfunction. These findings underline the need to assess tolerability and manage treatment-related risks alongside efficacy.
Upcoming LITESPARK-011 Analyses
At ESMO Congress 2026, the final LITESPARK-011 analysis will be presented as presentation #3587O. A separate rapid oral presentation, #3595RO, will examine the relationship between HRQoL and treatment efficacy.
PFS and OS measure disease progression and survival, while HRQoL can clarify how treatment affects patients’ daily lives and well-being. The forthcoming analysis may therefore provide additional context for interpreting clinical benefit beyond tumour response alone.
Eisai’s announcement has not yet disclosed the complete findings from these presentations. Their implications should be assessed after the detailed data become available.
LITESPARK-012: Investigating First-Line Combinations
LITESPARK-012 (NCT04736706) is a Phase 3, randomised, open-label study evaluating investigational regimens for patients receiving first-line treatment for advanced ccRCC.
The study compares pembrolizumab plus lenvatinib and belzutifan, and MK-1308A plus lenvatinib, with pembrolizumab plus lenvatinib. Findings are scheduled for presentation #3586O at ESMO Congress 2026.
Lenvatinib inhibits multiple receptor tyrosine kinases involved in angiogenesis and tumour growth, while belzutifan inhibits hypoxia-inducible factor-2 alpha (HIF-2α). Their complementary biological targets provide a rationale for combination treatment, but the clinical value of each regimen must be established through trial results.
The forthcoming LITESPARK-012 findings will be important for assessing whether the investigational combinations offer meaningful advantages over the active comparator. Efficacy and safety data are needed before conclusions can be drawn.
Clinical Interpretation
The LITESPARK programme addresses treatment decisions at different stages of advanced RCC. LITESPARK-011 has established a PFS advantage for belzutifan plus lenvatinib over cabozantinib in previously treated disease, supporting the FDA-approved option despite the absence of a statistically significant OS benefit. The safety profile remains relevant to treatment selection.
The upcoming LITESPARK-011 analyses may add information on longer-term outcomes and patient-reported quality of life, while LITESPARK-012 will address investigational first-line combinations. Until those detailed results are available, their clinical implications remain uncertain.
